Schaumburg, IL — July 5, 2010 — Community Home Support Services Association (CHSSA), a not-for-profit provider of in-home health care services to the elderly and individuals with physical and/or developmental disabilities in Western Canada, has deployed CellTrak’s communication system on BlackBerry® devices to improve patient care.

Previously, visits were recorded on paper and communications were achieved with voicemail, document copies and faxing. Keeping schedule and care plan changes up to date had became a problem for administrative and field staff. A time-consuming and inefficient way to exchange information, this practice led to communication breakdowns and missed appointments.

CHSSA and software partner Procura turned to CellTrak running on BlackBerry devices on the Rogers Wireless network and replaced most of their paper and manual processes. Community health workers now have all their daily information delivered in real time to their BlackBerry.

CHSSA reduced its paperwork and has seen a measurable improvement in data accuracy and visit compliance. Supervisors know the whereabouts of community health workers through GPS mapping, which helps ensure that the CHWs safely arrive at, and depart from, each appointment.

CHSSA employs  over 600 community health workers who provide  service in over 40 languages to approximately 2,200 clients in Vancouver and Burnaby, British Columbia.

Mundelein, IL, June 30, 2010 – To help home health care providers improve patient outcomes and comply with new federal regulations on data collection and best practices implementation, Medline Industries, Inc., and Strategic Healthcare Programs (SHP) are joining forces to provide a new reporting and benchmarking system that the companies describe as state-of-the-art. The automated program recognizes specific diagnosis codes in patient records and suggests best-practices supplies and interventions to achieve improved patient outcomes.

Medline and SHP developed the best practices technology to help home care providers comply with OASIS-C and to improve Home Health Compare scores.

Medline leverages SHP’s real-time reporting capabilities and automated data interfaces with the industry’s software vendors and transmits its patient-specific medical supply order data to SHP. Home health agencies receive SHP’s real-time email notifications alerting them to issues that require immediate attention. If the supply orders do not match best practice recommendations, action can be taken. SHP drill-down reports also identify the specific clinician delivering care, enabling targeted education and increased accountability.
